Our ​Doctors

President and CEO, Internist & Infectious Disease Specialist

Dr. Siddiqui is an Internist and Infectious Disease Specialist.

 He is board-certified in both Infectious Disease and Internal Medicine by the American Board of Internal Medicine (ABIM).

Graduated from Jinnah Sindh Medical University, Karachi, Pakistan (1986), and postgraduate diplomate (MRCP) from the Royal College of Physicians of Ireland (1997).

He completed U.S residency training in Internal Medicine from University of Iowa Hospitals & Clinics, Iowa City, IA, U.S.A (2001) and specialized in adult Infectious Diseases from Loyola University Medical Center, Maywood, IL, U.S.A (2003)

He also acquired postgraduate medical education and training from UK/Ireland by completing fellowship training in General Medicine/Pulmonary Diseases from Beaumont Hospital, affiliated with the Royal College of Surgeons of Ireland, Dublin. (1998), and practiced medicine for a decade in Ireland.

He is currently a practicing Infectious Disease specialist in Chicago, IL, in both the clinical as well as academic settings at multiple hospitals and holds various leadership positions including Chair, Infection Prevention & Department of Medicine, Clinical & Teaching Faculty of Family Medicine & Podiatry Residency Programs at Norwegian American Hospital, Chicago; Chair, Infection Control at Roseland Hospital, Chicago; Infectious Disease Specialist at St. Bernard Hospital, Chicago; Asst. Professor of Medicine at St. George University Medical School, and President/CEO of Chicago Infectious Disease Physicians Group.

Dr. Siddiqui has written several research articles, presented papers/posters at various scientific seminars and has delivered numerous CME lectures on a variety of topics pertinent to the local as well as global infectious diseases and HIV medicine.

In recognition of his professional achievements, dedication and continuous work, related to public health, medical education, scientific research, quality improvement, and patient care, he received many  awards from the national and international organizations, including The Marquis Who’s Who” Publication Board (Sixty Seventh Edition), Patient Reviews Top Doctor Certified, Consumers Research Council of America, Peer Reviewed professional, Philippine Medical Association in Chicago (PMAC), and  Pakistan Physicians Society in America.

Jaimie Cornwith, MD

Physican Assistant

President and CEO, Internist & Infectious Disease Specialist

Jaimie Cornwith is a nationally certified physician assistant who specializes in Infectious Disease at Norwegian American Hospital, Jackson Park Hospital and St Bernard Hospital. She completed her Bachelor of Arts in Biological Science from Wayne State University in Detroit, Michigan and then earned her Master of Medical Science, Physician Assistant Studies from Midwestern University in Downers Grove, Illinois. Jaimie Cornwith is a member of the American Academy of Physician Assistants (AAPA). She is certified by the National Commission on Certification of Physician Assistants.

She provides patient-centered care as the Physician Assistant for Dr. Tahseen J. Siddiqui. Jaimie Cornwith treats patients at Saint Bernard Ambulatory Clinic, Norwegian American Hospital Clinic and inpatient settings. She assists Dr. Tahseen J. Siddiqui in the treatment and management of a wide variety of infections including HIV and hepatitis care.

Jaimie Cornwith has one child. When Jaimie is not working, she enjoys traveling, working out and spending time with her family.

Meet Our Administrative Team

Sepas Siddiqui​

Assistant Practice Manager

Sepas Siddiqui helps organizing and facilitating practice's day to day operations by coordinating between patients and our healthcare team,  assisting Practice Manager in organizing and maintaining office records.  

As a patient's liaison officer, Sepas plays important role in developing and maintaining patient-provider relationship,  providing assistance to our patients with their appointments, clinical documentation, and follow ups.

He provides technical support for the electronic database entry and records into the EHR, and ensures implementation of quality measures.

Dr. Mohammad Y. Nomani

Practice Manager

Dr. Nomani possesses a vast experience in the field of medicine, both as a clinician and a health care administrator.

He obtained his medical degree from Sindh Medical College, University of Karachi, Pakistan.

As the chief practice administrator/manager, he organizes our busy clinics and in-patients services, and coordinates between our healthcare providers and patients.

As an experienced and compassionate patient's advocate, Dr. Nomani, helps and guides our patients with their clinic appointments, health insurance related matters, medication prescriptions and pre-authorizations, patient's referrals, as well as finding solutions and providing useful resources to benefit our patients.

Angela Blades 

Nursing and Patient Advocate

Angela is a registered nurse who developed a love for nursing in her early teens. She migrated from Trinidad and Tobago in 1994. Angela attended Kennedy King College where she earned a degree in nursing.

As a nurse at the University of Chicago. Angela gained useful experience, which later benefited her nursing career. She has a wide variety of nursing experience as a Medical Surgical nurse, Cardiac/Tele nurse and a nurse on the Postpartum and Labor and delivery units, before accepting a position at the Outpatient Clinic at St Bernard Hospital where she has worked for the past four years.

Angela currently handles the operations of our clinic and is able to facilitate the smooth transition and coordination of care between patients physicians and staff.

She enjoys going to church, singing, traveling and spending time with her two grandchildren.
